Ronald McDonald House Pittsburgh Morgantown Volunteer Release
I hereby grant to Ronald McDonald House Pittsburgh Morgantown, its local Chapters and programs, advertising and promotional agencies, and their agents (collectively, “Ronald McDonald House”) the irrevocable, unrestricted right to use, publish, display and distribute materials bearing my name, voice, likeness or any other identifiable representation of myself. These materials may appear in any form, style, color, or medium whatsoever now or hereafter known *including, without limitation, photographs, videotapes, films, sound recordings, software, drawings, prints, broadcast, internet (including social media sites like Facebook, Instagram and Twitter), and electronic media).
I agree that all materials containing any identifiable representation of me (including, without limitation, all negatives, plates, and masters of any photographs, files, prints, or tapes) shall be and remain the sole and exclusive property of Ronald McDonald House, and I hereby assign any proprietary right I may have acquired in or to such material to Ronald McDonald House.
I hereby release and forever discharge Ronald McDonald House Pittsburgh Morgantown from any and all liability and damages relating to the use of my name, voice, likeness, or any other identifiable representation of me.
I hereby waive any right I may have to inspect or approve the finished materials or any part or element thereof that incorporates my name, voice, likeness, or any other identifiable representation of myself.
I have agreed to the above in consideration of the opportunity given to me by Ronald McDonald House Pittsburgh Morgantown to appear in these materials. I acknowledge that I have fully read and understand this document and that I have had any questions regarding its effect or the meaning of its terms answered to my satisfaction. I certify that I am at least 18 years of age, unless this document is also signed by my parent or legal guardian.
Ronald McDonald House Pittsburgh Morgantown promotes the privacy and confidentiality of information related to our guest families. I understand that I must maintain the confidentiality of Ronald McDonald House guests. Volunteers must not disclose confidential information or use the names, photos or other identifying characteristics of RMHC guest families in order to respect the privacy of other families and be aware of appropriate use of social media outlets.
I agree that I am at Ronald McDonald House Pittsburgh Morgantown voluntarily and understand that Ronald McDonald House, its staff, volunteers and others associated with Ronald McDonald House will not be responsible or liable under any theory of liability for any claims, losses, damages or injuries of any kind or nature, including, without limitation, any loss of or damage to my valuables, or other personal property from any cause, including Ronald McDonald House’s negligence, and/or for any personal injuries, illness, or death to us (or any of us) from any cause, including Ronald McDonald House’s negligence. I agree to indemnify, defend and hold Ronald McDonald House (and employees, agents, affiliates, and their respective heirs, personal representatives, successors and assigns) harmless from any and all claims, damages, losses, costs and expenses (including reasonable attorneys’ fees) arising from any personal injuries, illness, or death from any cause, including, without limitation: Ronald McDonald House’s negligence. I agree that Ronald McDonald House is not responsible for personal belongings left in the House, on House property, or in automobiles. I waive any and all rights to claims in the event of damage, theft, or loss.
